The COVID-19 Vaccine Clinic at Baylor Scott & White Health and Wellness Center, BSW HWC at Juanita J. Craft Recreation Center is open on Tuesday and Wednesday.

Location: 4500 Spring Avenue, Dallas, Texas 75210

Tuesday, 9:00 AM-3:00 PM, and Wednesday, 9:00 AM-12:00 PM.

COVID-19 Vaccine Clinic details:

• Pfizer vaccine only

• 1st, 2nd, 3rd or booster dose available

• Pre-registration required

• Baylor Scott & White Health consent forms completed on-site

• Youth ages 12-17 (must be accompanied by a parent/legal guardian)

• Adults ages 18 and older

Please bring a vaccine record of previous doses.
